Should you make a photo?

Do not prepare your own photo unless the agency or your transaction instructions specifically ask for one.

Where the photo is taken

Yes. To get an instruction permit you go to a DMV office, where DMV takes your picture; you leave with an interim card and the permit itself is mailed to you.

Online steps and photo upload

You can take the knowledge test online on a computer with a webcam, but the permit photo is still taken at the DMV office. The DMV page does not mention uploading your own photo.
